ASCOT is free of charge for not-for-profit use, but a licence is required.

Please complete Form 1 for not-for-profit use of:

• ASCOT-SCT4 or INT4, ASCOT-Proxy, ASCOT-ER, ASCOT-ER(OP), ASCOT-Carer SCT4 or INT4 and ASCOT-Workforce

Form 1 is automated. You will receive a copy of the measure and guidance.

Please complete Form 2 for not-for-profit use of:

• ASCOT-CH4 or ASCOT application in care planning, which require training
• ASCOT Control over daily life in the ICHOM Older Person Standard Set
• ASCOT translations not available from in-country ASCOT teams (see Translations)
• Requests to amend or adapt any ASCOT tool

Form 2 requests review/approval from the University of Kent.

For-profit licences

If you wish to apply for a licence for for-profit use of ASCOT (any tool), please complete Form 3.

The pricing structure for a for-profit licence is outlined below

The application is reviewed by an ASCOT team member, who will then forward your application to Research and Innovation Services, for costing and contracting.

It may take at least 10 working days for a licence to be issued by UoK Research and Innovation Services.
